Minutes of 8/27/04 meeting: Osseointegration Study Club
of
Meeting started at 8:45 AM
Introduction: Roy
Yanase. Welcome to the lab tech present who are a major portion of the treatment
team
Speaker 1: Chris Marchack, DDS,
Presented an update on new generation Procera scanning technology. The NobelRondo Porcelain designed for Zirconia was presented. Tooth in an hour: A video demonstrated the step by step process.
Speaker 2:
Bernard Wu, DDS, UCLA,
Is there more life than BMP. FDA approved for spinal fusion. Today, there are 15 BMP’s identified. BMP activity depends on carrier chemical nature. Orthopedic applications and animal research were reviewed. BMP coated grafting material were introduced. A vision for chairside bone graft engineering was shared.
Speaker 3:
William Mihram, DDS,
Accelerated osteogenic orthodontics. Arch expansion and augmentation. By performing corticotomies of the alveolar process with a round bur, orthodontic treatment time can be shortened significantly.
Speaker 4: Momo
Vasilic, MDT, NobelBiocare,
NewProcera Esthetics. New techniques available from NobelBiocare can help by simplifying treatment steps for both clinician and technician. Silanization may help bonding of esthetic restorative materials to metals. This is still controversial and further studies should be done to verify or validate this technique.
Speaker 5: Yoko
Kimura, DDS,
Implant treatment for the edentulous maxilla. Implant failure rate restored with overdenture is about 20%. YK reviewed essential prosthetic parameters allowing the prescription of fixed versus removable prosthesis in the maxilla. With 15 mm occlusal clearance, the use a substructure is advised with cemented restoration.
Speaker 6: Jun-Ichi
Kudo, DDS,
Case report of an edentulous patient with gagging reflex.
Using state of the art (in office) technology including Newtom scanner and the
Simplant software, flapless surgery, immediately loaded fixed partial dentures
using Procera frameworks were provided to this patient.
The uniqueness of Dr. Kudo’s treatment is the use of only 4 implants
sized 5 X 10 mm. A Procera PIB (
Speaker 7:
Seiichiro Kinjo, DDS,
Usage of osteotome in sinus floor elevation. Several steps using osteotomes were presented: apical condensation, buccal expansion, cancelous bone condensation, development of implant site, elevation of sinus floor. Using a modified sinus graft with BioOss, Astra implant system, the highly predictable clinical outcome was validated using resonance frequency analysis (ISQ values).
Speaker 8:
Kiyokata Umezu,
Implant treatment in the esthetic zone. Treatment with tomograms, wax-up, surgical guide, custom abutment, orthodontics contributed to predictable treatment outcomes. A thorough review of the existing literature regarding the depth of implant placement was presented. Case reports of 47 restored implants revealed that the keratinized gingiva was greater that on teeth; the interdental papilla height was proportionate to the interproximal bone present. Interproximal bone was enhanced with orthodontic extrusion in selected cases.
Speaker 9: Sasha
Jovanovic, DDS, UCLA,
Presentation of a live lecture on the internet (by LiveWeb) also available on DVD. This new technology includes two-way audio communication and chat room for up to 40 people and a slide presentation. A series of lecture are available through LiveWeb.
SJ gave a lecture sample on “natural implant aesthetics”. Indications and technique for the new scalloped NobelBiocare implants to maintain interproximal soft tissues between two implants or one implant and two teeth. The transmission was interrupted because of technical difficulties…Orthodontic forced eruption can be used for esthetic enhancement. The NobelDirect implant technique was presented.
The technology does not support live video yet.
Speaker 10:
Toshiro Yamada, DDS,
Update on Profile prosthesis: lest costly than complex implant fixed restorations involving bone graft to correct deficient anatomy. Using pink porcelain for gingiva, acrylic or porcelain teeth, straight or angled abutments and using Procera titanium framework, the profile prosthesis can be adapted to multiple anatomical conditions. Several cases of full and partial edentulism were presented.
Speaker 11: Roy
Yanase, DDS.
20
years follow up on the “two first” patients. Evidence of long-term success
of implants is well accepted today. Considerations of implant design, patient
characteristic were discussed. Patient # 1: the onlyomplications was one broken
screw (at 10 years). Patient # 2 had previously received 10 year earlier a
subperiosteal implant (removed in 1983). She received 5 implants in the mandible
(1984). At 10 years some prosthetic screws either loosened or fractured and were
changed subsequently. This patient received then an upper denture retained by a
3 implant bar. The implants failed. She received later a bone graft of the
maxillary ridge and 8 implants to anchor a maxillary overdenture. She then
received a fixed complete denture on 8 implants. She is still functioning well
in 2004.
New
material and techniques will able to perhaps extend our vision of denture
forever…
